First visit checklist

1. Schedule an appointment

Schedule your first appointment online now. We often have same-day, evening, and weekend availability.

If you cannot find a spot soon enough, be sure to use the waitlist feature on the bottom left of the booking page. You will then receive notifications when something in your availability opens up.

2. Confirm insurance coverage

Insurance coverage varies by practitioner. See our Insurance & coverage page for the chart of which practitioners accept your plan, and for what to expect with copays and deductibles.

Our office staff will do their best to confirm benefits prior to your visit. Please feel free to call for further clarification.

3. Complete new patient intake

After scheduling your first appointment, you will receive our New Patient Intake Form via email. Please complete it no later than 48 hours before your appointment time.

4. Note our 24 hour cancellation policy

We require 24 hours notice to cancel or change appointments, and changes with less notice are charged. Please read our cancellation policy before your first visit.
