Consumer Health Data Privacy Policy
This policy is for Washington residents, and for anyone whose information is collected in Washington, under the Washington My Health My Data Act. It covers the mandalamed.com website. Records of your care are covered by our Notice of Privacy Practices instead, which you receive at the clinic and can ask for at any time.
The consumer health data we may collect
- That your browser visited a page about a health service or a practitioner, or tapped to book or call, through Google Analytics. Visitors in Washington are measured only after they choose Allow.
- Anything about your health that you choose to type into the contact form or the website guide. We ask you not to.
Why we collect it
To learn which pages help people find care, and to answer the questions you send us. We do not use it for advertising.
Where it comes from
From you, and from your browser while you use the website.
Who we share it with
- Google, for analytics (only after you choose Allow).
- Ulric.Studio, which built and runs the custom website system, its open source AI model chain and its security, for what you type into the website guide.
- Our email service, which sends the emails from our lists.
We do not sell consumer health data, and we have no affiliates.
Your rights
You can ask to see the consumer health data we hold about you and the list of everyone we shared it with, ask us to delete it, and withdraw your consent at any time. The Privacy choices link at the bottom of every page turns analytics off straight away. To make a request, call 503-758-9760 or write to [email protected]. We answer within 45 days. If a request is unusually complex we may take up to 45 more days, and we will tell you why within the first 45.
Appeals
If we turn down your request, you can appeal by writing to [email protected] with the word Appeal in the subject line, and we answer the appeal in writing within 45 days. If the appeal is denied, you can contact the Washington State Attorney General.
