Insurance & coverage

We accept insurance for all modalities at Mandala and bill to most major insurance companies. However, each practitioner is independent and therefore not all practitioners take all insurances. Please consult the chart below to confirm that the provider you are seeing is in your network.

Checking your benefits

Our office staff will do their best to confirm benefits prior to your visit. We are happy to check on your benefits for you: call our office at 503-758-9760, or fill in your insurance details on the form when you book your first visit online.

Copay or coinsurance

Most visits have a copay or coinsurance between $10 and $50 per visit.

Patient responsibility

Benefits quoted to us are subject to your plan limits at the time of service, and patients are responsible for the cost of treatment in the event that insurance does not end up covering services. Your benefits may also be subject to a deductible prior to your copay or coinsurance.

We always recommend patients confirm their coverage for themselves, as we can only go off of information relayed to us by your coverage reps, who can be incorrect.

After a car or bike accident

PIP coverage typically pays for 100% of acupuncture and massage treatments related to your motor vehicle accident. See Car accident care for how it works here.
